Website privacy
What this website collects, why, and who sees it. This notice covers the website and the matching team only. Your clinical records are kept by the independent therapist who treats you, under their own Notice of Privacy Practices.
What we collect
When you send the assessment or a call-back request, we receive what you typed: your name, phone number, email address, ZIP code, and your answers about the problem you want help with. We also record the page you sent it from and, if present, the referring website or advertising tags, so we know which pages are useful.
What we use it for
To match you with a physical therapist in the network, contact you about your enquiry, and prepare for that conversation. We share your questionnaire with the one therapist we match you with, so their office can confirm your benefits and book you. We do not sell your information or share it with advertisers.
Who handles it
Submissions are delivered to the matching team by email (through Resend, an email delivery service) and kept in a secure backup database (hosted on Supabase) so that no enquiry is lost. The website runs on Cloudflare. Email alerts to the matching team carry your contact details and scheduling preferences only; your answers about your health stay in the backup database. We keep enquiries for as long as needed to match you and follow up, and delete them on request. No advertising pixels or analytics run on the questionnaire. Every physical therapist in the network is an independent practice, licensed in Indiana and responsible for your evaluation, treatment, records and billing. The network matches and coordinates; it does not provide healthcare and does not employ therapists. Please do not include Social Security numbers, insurance member IDs or payment details in any website form. If the patient is under 18, a parent or legal guardian must complete the form.
Calls and text messages
If you tick the consent box, you agree that the matching team and the independent therapist practice we match you with may call or text you at the number you give, about your enquiry, matching, scheduling, benefits confirmation and appointments. Calls come from a person on the matching team or at the therapist’s office. Message frequency varies. Message and data rates may apply. Reply STOP to opt out of texts or HELP for help, and tell us at any time if you want no further calls. Consent is not a condition of receiving care or buying anything.
Third-party content
The reviews shown on this site are loaded from Elfsight and Google, and the map on the contact page is loaded from Google Maps. Those services may set their own cookies.
