Most Anthem Blue Cross and Blue Shield plans in Indiana include outpatient physical therapy, but the deductible, copay, visit allowance and referral rules come from your specific plan, not from Anthem as a whole. An employer PPO, a Marketplace plan, an Anthem Medicare Advantage plan and an Anthem Medicaid plan can treat the same therapy visit very differently. The matching team asks which Anthem plan you have, and the therapist’s office verifies your benefits before the first visit.

Which Anthem plan do you have, and why does it change your therapy coverage?
Anthem sells several kinds of coverage in Indiana, and each follows a different rulebook. The name on your card is the same, but an employer plan follows the contract your employer chose, a Marketplace plan follows federal essential-benefit rules, a Medicare Advantage plan follows Medicare rules, and a Medicaid plan follows Indiana Medicaid’s rules.
Start by finding the plan line on your card or in the Sydney Health app, which Anthem offers for checking benefits and ID cards. Then use the table below to see where your physical therapy rules come from. If you are not sure what any of the cost terms mean, copay versus coinsurance for physical therapy walks through them.
| Anthem plan line | Where the therapy rules come from | What to look for |
|---|---|---|
| Employer plan (small group, large group, national account) | Your employer’s benefit booklet | Rehab visit allowance, deductible, copay or coinsurance |
| Individual or Marketplace plan | Federal essential health benefits plus your plan’s schedule | Rehabilitative services line in the Summary of Benefits |
| Medicare Advantage or Dual Special Needs Plan | Medicare rules plus the plan’s network and approvals | Specialist copay, referral and prior authorization rules |
| Medicaid (Indiana programs) | Indiana Medicaid rules | See Medicaid and the Healthy Indiana Plan |
How do Anthem deductibles and visit allowances apply to physical therapy?
On most commercial Anthem plans, physical therapy is billed as an outpatient rehabilitative service with its own line in the Summary of Benefits and Coverage. That line tells you whether visits cost a flat copay or a percentage after the deductible, and whether there is a yearly visit allowance.
Two details trip people up. Some plans count physical, occupational and speech therapy visits against one shared allowance, and the evaluation visit can be billed differently from follow-up sessions. Ask about both. For a fuller explanation of yearly limits, see how many physical therapy visits insurance covers.
Does an Anthem plan require a referral or prior approval for physical therapy?
It depends on the plan type, not the state. Indiana law lets a physical therapist treat you for up to 42 calendar days without a referral, but an Anthem HMO or EPO plan can still require you to stay in its network, and some plans ask for a primary care referral or prior authorization before paying.
HealthCare.gov describes preauthorization as the insurer’s decision that a service is medically necessary, and notes it does not promise payment. On many plans the therapist’s office requests any approval after the evaluation. If yours does, the prior authorization explainer shows what usually happens next.
What changed for IU Health Plans members who moved to Anthem in 2026?
On January 1, 2026, Indiana University Health Plans became Anthem Blue Cross and Blue Shield. That affected Medicare Advantage members and members of employer-sponsored plans that IU Health Plans had administered.
If your therapy started under an IU Health Plans card, check that the therapist’s office has your current Anthem member details and whether any earlier approval still stands. The IU Health Plans page lists the Anthem phone lines for each group.
How does the matching team use your Anthem details?
In the questionnaire, name Anthem and, if you know it, the plan line. A person on the matching team reads it and proposes an independent, Indiana-licensed therapist whose office works with your Anthem plan where that is possible, or tells you if it is not. Nobody in the network can confirm your benefits; the therapist’s office does that before your first visit.
Sessions take place one-on-one in private suites inside FlexWerk at Carmel City Center, open from 5 AM on weekdays. Have your member ID ready when the therapist’s office calls, and start with the two-minute questionnaire.
